Transition II is a private, non-profit organization that has been invested in supporting Vermonters since 1981.
We currently serve Vermonters through three service options:
We are passionate about collaborating with each participant we partner with to help them facilitate their service option in a way that will promote a greater quality of life, greater independence, employment opportunities, community involvement, and home care choices.
Transition II is committed to working with participants in a creative, person-centered manner so that their specific and unique needs are met. We aim to be respectful, flexible, and objective in our partnership with those we serve.
