Self-Directed Services Options

Individuals with Intellectual or Developmental Disabilities

Vermonters with developmental disabilities who are Home and Community-Based Services Medicaid recipients, and their families, can choose to fully self or family-manage their services with our support.  Transition II provides orientation, training and guidance on how to manage services successfully. Through self and family-management, individuals can hire their own support workers, manage their budgets and follow Medicaid and State guidelines.  Learn more…

Elderly Vermonters and those with Physical Disabilities

Flexible Choices, an option for Vermonters who are elderly or with physical disabilities, provides opportunities to use funding in new ways that help keep people more healthy, safe, and independent.  It is designed for people who are currently enrolled in the Choices for Care long-term care Medicaid program.  Transition II provides support by offering orientation, training and guidance on hiring support workers, managing participant service budgets, and following Federal and State guidelines.  Learn more…
